It will milk you whether you know it or not. I wear a pullup or diaper over it to keep it snug, but also catch anything that comes out. After about a half hour of doing routine activities I pulled open my diaper to find a nice puddle sitting in the front. It definitely does the job! The part that sits outside between the legs can start to rub and be uncomfortable, especially if you walking around with it.
